Integrated Care Management Initiative: Summary of Lessons Learned

In 2004, and again in 2005, the Administration on Aging (AoA) announced the Integrated Care Management Grant Program with the intention of understanding how the strengths of the Aging Services Network can add value to models of integrated health and long term care. AoA asked NCOA to conduct an assessment of this program, which is described in the following report.
