The Model Programs Project

Using the Evidence Base to Promote Healthy Aging- Issue Brief 2 Spring 2005

This issue brief is the second in the evidence-based health promotion series by the National Council on the Aging's (NCOA) Center for Healthy Aging. It focuses on the NCOA Model Programs Project that included the development and testing of four evidence-based model health promotion programs that can improve the health of older adults and be readily implemented by community-based aging services organizations.

Each of the model programs, now being widely disseminated, addresses an important health promotion topic: physical activity, depression, diabetes self-management, or nutrition. Each program has produced an easy to use toolkit that includes program materials, an explanation of the evidence that supports the program's effectiveness, guidelines on preparing your organization to implement evidence based programming, and methods for developing effective partnerships to sustain program efforts.
