Medication Management
Older adults are the nation's largest consumers of prescription and over-the-counter medications. On average, the older adult takes 4-5 prescription medications and 2 over-the-counter medications a day. Given the number of medications and older person takes and the biological declines one undergoes with age, this population is especially vulnerable to medication-related problems. The Medication Management Model has proven to be an effective intervention for reducing medication-related problems.
